Osteoarthritis (OA) is a degenerative joint condition characterised by the progressive breakdown of articular cartilage at affected joint surfaces. It commonly affects the knees, hips, hands, and spine. Symptoms can vary widely, with some people noticing mild discomfort after activity, while others may experience more disruptive symptoms that interfere with work, sleep, or exercise.
Although there is currently no cure for OA, a range of treatments can help manage symptoms and slow progression. Among these, weight loss is a well-established non-surgical approach. Reducing stress on weight-bearing joints can help alleviate pain, improve mobility, and support better long-term joint health.
This article explores how weight loss influences osteoarthritis, the mechanisms behind its benefits, and practical approaches to incorporating it into an effective management plan.
What Is Osteoarthritis?
Osteoarthritis primarily affects weight-bearing joints, including the knees, hips, lumbar spine, and ankles, though involvement of the hand joints is also commonly observed. It typically presents with:
- Joint pain, especially during or after movement
- Stiffness, often noticeable after periods of rest or in the morning
- Reduced mobility, making it harder to move the joint through its full range of motion
The condition occurs when cartilage, the smooth tissue that allows joints to move freely, begins to break down. Once cartilage is lost, the bones on either side of the joint start to rub against each other, causing pain, swelling, and inflammation. Without proper management, this can lead to further joint damage and a decline in physical function.
Risk Factors of Osteoarthritis
These are factors that can increase the likelihood of developing or worsening osteoarthritis over time.
- Age: The likelihood of developing OA increases with age, particularly after 40. Over time, joint cartilage gradually wears down and becomes less able to repair itself, while surrounding muscles may weaken.
- Joint injuries: Previous injuries, such as ligament tears, fractures, or sports-related trauma, can raise the risk of OA, even if they occurred many years earlier.
- Obesity: Body weight is a risk factor for OA not only in the weight-bearing joints, including the knee and hip, but also in the hand. Excess weight places additional stress on weight-bearing joints like the knees and hips, while fat tissue may produce substances that promote joint inflammation.
- Sex: OA of the hands and knees is more common in women than men, while hip OA is equally prevalent. It is more common in the postmenopausal years, due to either age or loss of oestrogen.
- Genetics: A family history of OA can increase risk, as some people inherit a tendency for joint degeneration or cartilage defects.
- Anatomical factors: Joint shape, particularly of the hip, can influence the development of OA. Conditions such as congenital acetabular dysplasia are also associated with premature hip OA.
- Repetitive joint stress: Occupations or activities that involve frequent kneeling, squatting, or repetitive movement can increase wear and tear on joints over time.
- Sedentary lifestyle: Lack of physical activity can weaken the muscles supporting the joints, leading to reduced stability and increased strain.
- Pre-existing joint conditions: Conditions such as rheumatoid arthritis or gout can damage joints and increase the risk of developing OA.
- Metabolic diseases: Disorders such as diabetes, conditions involving excess iron (e.g., hemochromatosis), or abnormal hormone levels can contribute to joint damage.
The Stages of Osteoarthritis
Osteoarthritis progresses gradually and is generally divided into four stages, based on how much the joint has deteriorated over time:
Stage 1: Early Stage
At this point, only minimal damage has occurred. The cartilage may show slight wear, but the space between the joints is still largely intact.
Symptoms are usually subtle, with occasional discomfort or mild pain after activity. Imaging tests like X-rays often appear normal.
Stage 2: Mild Stage
Cartilage breakdown becomes more noticeable, and the joint space may begin to narrow slightly. You may start to feel more consistent pain, stiffness, or reduced flexibility, especially after movement or exercise. Early structural changes may now be visible on X-rays.
Stage 3: Moderate Stage
By this stage, cartilage loss is more significant, leading to increased friction in the joint. Pain and stiffness are more persistent and can interfere with everyday movements such as bending, squatting, or climbing stairs. Bone spurs may form, and imaging typically shows clear joint damage.
Stage 4: Advanced Stage
This is the most severe stage, where cartilage is greatly reduced or nearly absent. The joint may become deformed, and large bone spurs can develop.
Pain is often constant, even at rest, and mobility is severely limited, making daily tasks difficult. X-rays usually reveal extensive joint deterioration.
How Excess Weight Worsens Osteoarthritis
Carrying excess body weight places additional mechanical stress on the joints, accelerating wear and tear over time. Excess weight also significantly raises the risk of developing OA. According to studies, overweight women are approximately four times more likely to develop knee OA, while overweight men face around five times the risk compared to those at a healthy weight.1
It is important to note that OA is not purely a mechanical problem. Obesity is associated with systemic inflammation, which can affect joints throughout the body, including those that do not bear weight, such as the joints in the hands.
Increased Joint Load
Research shows that every pound (0.45kg) of body weight adds approximately four pounds (1.81kg) of pressure on the knees.2 During everyday activities, the load on joints increases further. Walking places around 1.5 times a person’s body weight on the knees, and this rises to two to three times body weight when walking on an incline.3
The more load a joint carries, the faster cartilage tends to break down. In individuals with OA, joint misalignment, which is common as the condition progresses, further amplifies the stress placed on already damaged joint surfaces, accelerating deterioration.
Inflammation
Fat tissue is metabolically active and produces inflammatory chemicals that circulate throughout the body. This low-grade, chronic inflammation can worsen OA symptoms and contribute to joint damage across multiple sites, not just the weight-bearing joints.
Cartilage Degeneration
The combination of increased mechanical load and ongoing inflammation accelerates cartilage loss. Those with higher body weight may experience faster disease progression, making early weight management an important part of slowing the condition.
How to Manage Osteoarthritis
The overarching aims of osteoarthritis management are to minimise pain, optimise function, and slow the progression of joint damage. As individual treatments often provide only modest benefits on their own, your doctor will typically tailor a combination of therapies to suit your specific needs and condition. This personalised approach takes into account the key factors contributing to your symptoms, such as joint malalignment, muscle weakness, excess weight, and concurrent depression.
Lifestyle interventions form the foundation of OA management and serve as the first line of treatment. Of these, exercise and weight loss are the most well-established and can make a meaningful difference in reducing pain and improving mobility, and potentially in slowing the disease itself.
When lifestyle measures alone are not enough, your doctor may recommend pharmacological treatment alongside them to provide additional relief.
Benefits of Weight Loss for Osteoarthritis
There are many benefits of weight loss for individuals with osteoarthritis. Even modest reductions in body weight can produce meaningful improvements in osteoarthritis symptoms and joint health.
Pain Reduction
Losing as little as 5% of body weight has been shown to significantly reduce joint pain. Greater weight loss of around 10% may reduce pain by up to 50%, offering substantial relief for those with osteoarthritis.4
Reduced Joint Stress
The reduction in joint load with weight loss is considerable. Losing just five pounds (2.27kg) removes approximately 20 pounds (9.07kg) of pressure from the knees2, which can make a noticeable difference in day-to-day comfort and mobility.
Slower Disease Progression
Weight loss may help preserve remaining cartilage and delay the worsening of OA. Studies suggest that losing approximately 11 pounds (4.99kg), equivalent to around two BMI units, can reduce the risk of knee OA progression by more than 50%.1
Improved Quality of Life
As pain decreases and joint function improves, individuals with OA often experience greater independence in daily activities. In some cases, sustained weight loss may also reduce the likelihood of requiring joint surgery in the future.
Who Should Consider Weight Loss?
Body Mass Index (BMI) is a commonly used tool to assess whether a person’s weight may be affecting their health. BMI categories are as follows:
- >27.4: Obese
- 23.0–27.4: Overweight
- 18.5–22.9: Normal
- <18.5: Underweight
Note: These thresholds follow Asian BMI guidelines and may differ for other ethnic groups.
Waist circumference is another useful indicator, as excess abdominal fat is associated with higher levels of systemic inflammation and increased health risk.
People with osteoarthritis who fall into the overweight or obese category are likely to benefit from weight loss as part of their overall treatment plan. Even small reductions in weight can have a positive impact on symptoms and joint health.
Practical Weight Loss Strategies for Osteoarthritis Management
Managing weight with osteoarthritis requires a balanced, sustainable approach. Rather than pursuing short-term or restrictive diets, the focus should be on long-term lifestyle changes that support gradual, steady weight loss and tackle joint pain and reduced mobility. There are a variety of ways to achieve weight loss, including diet, exercise, pharmacotherapy, surgery, or a combination of these.
Dietary Changes
Reducing overall calorie intake is a key step in achieving weight loss. A well-balanced diet should include:
- Vegetables, fruits, and whole grains
- Lean protein sources, e.g., fish, poultry, legumes, and low-fat dairy
- Healthy fats, e.g., nuts, seeds, and olive oil
At the same time, you should aim to limit:
- Sugary drinks
- Ultra-processed foods
- Foods high in saturated fats
Physical Activity
Regular physical activity plays an important role in both weight management and joint health. Low-impact exercises are generally well-tolerated in those with OA and include walking, cycling, and swimming. These activities help burn calories while minimising stress on the joints.
Strengthening exercises that target the muscles around affected joints, particularly the knees and hips, can help improve stability and reduce pain. Flexibility exercises such as stretching and yoga can also support joint mobility and overall function. These types of exercises are important for bone health, helping to maintain bone density and reduce the risk of osteoporosis, another related condition in older adults.
Behavioural Strategies
Setting realistic, achievable goals is important for long-term success. A target of 5–10% body weight loss is a reasonable starting point for those with OA. A gradual rate of weight loss, around 0.5 to 1kg per week, is generally recommended as it is more sustainable and reduces the risk of muscle loss.
Keeping track of food intake and monitoring progress over time can help you stay on track and make adjustments as needed. Working with a healthcare professional or dietitian can also provide structured support throughout the process.
Pharmacotherapy
GLP-1 receptor agonists, for example, semaglutide, and other anti-obesity medications are being actively studied as treatments for knee osteoarthritis (OA).
In one clinical trial involving patients with obesity and moderate knee osteoarthritis, semaglutide (2.4 mg once weekly) was shown to produce greater weight loss and more improvement in knee pain compared to a placebo (inactive substance with no effect on the disease).5 However, it is important to note that participants in the placebo group also experienced meaningful improvements through diet changes and exercise counselling alone.
Medication may be appropriate for some patients after medical assessment, but lifestyle changes remain the foundation of treatment. Each individual’s symptoms, health status, and contributing factors greatly influence the uniqueness of their treatment plan.
Bariatric Surgery
Surgical approaches to weight loss, such as bariatric surgery, have been studied less extensively. While they may be appropriate in selected individuals with obesity and related health risks, they are not routine solutions for everyone with knee symptoms.

Maintaining a healthy weight is one of the most effective steps, as it reduces the long-term load placed on your joints. Regular physical activity that strengthens the muscles around your joints can also offer protection, as can avoiding repeated joint injuries and practising good posture. While these measures do not eliminate the risk, they can lower the likelihood of developing OA later in life.
7. Can I stop osteoarthritis from getting worse?
While osteoarthritis cannot be reversed, its progression can be slowed. Losing weight if you are overweight, staying active with low-impact exercise, and following your treatment plan are among the most effective steps you can take. With consistent self-management and appropriate medical support, you can maintain a good quality of life.
8. Are weight loss medications suitable for people with osteoarthritis?
In selected patients, weight management medications may be considered after a medical assessment. Suitability depends on your overall health, weight-related risks, and treatment goals.
