What is Shoulder Impingement?

Understanding Shoulder Impingement with Dr. Sean Leo.

Shoulder impingement occurs when the tendons of the rotator cuff muscles become irritated and inflamed as they pass through the subacromial space—the narrow passage between the top of your arm bone and the tip of your shoulder (acromion). This is often caused by bone spurs, which are bony projections that develop along the edges of bones, further narrowing the space and pinching the tendons.

What are the Symptoms of an Impingement/Bone Spurs?

Shoulder impingement or bone spurs often cause pain when lifting your arm overhead or reaching behind your back, along with a dull ache that can worsen at night. You may also experience arm weakness and stiffness, and the pain may radiate from the front of the shoulder down the side of the arm.

Pain when lifting your arm overhead or reaching behind your back

This type of pain often occurs when shoulder structures become compressed or irritated, making everyday movements like reaching, lifting, or dressing uncomfortable.

A dull ache in the shoulder that worsens at night

A persistent, aching discomfort may intensify when lying down, especially on the affected side, due to increased pressure on the shoulder.

Weakness and stiffness in the arm

You may notice reduced strength and limited mobility, making it harder to lift or rotate the arm during normal activities.

Pain that radiates from the front of the shoulder to the side of the arm

This symptom often signals inflammation or compression within the shoulder joint, causing pain to spread along the outer arm.

Bone Impingement/Spurs
Treatment and Recovery

Treatment focuses on creating more space for the rotator cuff tendons.
  • Non-Surgical Treatments
  • Surgical Treatments

What are the treatment options for Impingement/Spurs?

Surgical vs Non-surgical
  • Activity Modification: Avoiding activities that cause pain.
  • Physiotherapy: To strengthen the rotator cuff and improve shoulder mechanics.
  • Anti-inflammatory Medication & Injections: To reduce pain and swelling.

When would you need surgery?

Act Promptly for Better Outcomes
  • Arthroscopic Subacromial Decompression: This is a common minimally invasive surgery. The surgeon removes inflamed tissue and shaves away the bone spur and a small part of the acromion to create more space for the tendon.

Recovery from surgery involves a period of rest followed by a structured physiotherapy program to regain strength and motion, typically over several months.
